New Patient

Thank you for choosing your locally-owned Good Value Pharmacy! To get started with us, all you have to do is fill out the New Patient Form and bring it in to one of our convenient locations. You can also fax the form to us using the fax numbers listed on the form. Or just give us a call and we can do it all over the phone!

To transfer your prescriptions to us from another pharmacy, just fill out this form online! Alternatively, you can have your doctor send us the prescriptions directly.

A copy of our Notice of Privacy Practices is available which contains information regarding Good Value Pharmacy’s use and disclosure of your personal health information.
